Pathology
Schwannomas are the most common intradural extramedullary spinal lesions (30% cases), followed by meningiomas ( 25% cases).
MRI
MRI is the modality of choice to fully characterise these masses, and the appearance will vary depending on the histology. The key benefits of MRI over other modalities, is the ability to clearly define the relationship of the mass to the cord, to identify secondary lesions, to identify large feeding/draining vessels and hopefully to give a pre-operative diagnosis.
Treatment
The recommended operation for the IDEM is middline posterior approach in prone position. After laminectomy dura is opened in the midline and tumor can be approache from the psterion approach. 40 years old patient presented in hospital with complain of weakness of all four limbs for last 3 months. the weakness was progressive. patient was whell chair bound for 2 months. Patient also had difficulty in passing urine. On examination patient was having spastic quadreparesis. Power in limbs were 3/5.
